Create your evaluation certification with whatever fields you want the supervisor to complete. You should include a field like Supervisor e-mail: doing this lets you send the evaluation requests to both people in VSys as well as those who are not. (The certification below is just an example.)
You might want to enable an automatic expiration date for these or not, and most of the other settings aren't important to how we're using this certification.
