Commercial Investigations background checks ("Origin") don't require any details about the volunteer. When submitting to Commercial Investigations the volunteer's details are culled from their profile automatically. The only field you'd edit in the request here is the Status field: set it to "Ready to send" if you want it submitted to Commercial Investigations the next time requests are sent, or to "On hold" or "Cancelled" to prevent its being sent.
